The death card is issued by the doctor who confirmed the death. It is free and unlocks two things: the burial (a part for the cemetery administration) and the registration of the death at the civil registry office. The death certificate is drawn up by the registrar on the basis of the card. Only a copy of the certificate is the document a bank, insurer, ZUS, notary or court will ask for. The order never changes: card first, certificate second.
