After registering the death the registrar issues one short-form copy free of charge. Further copies cost PLN 22 (short form) and PLN 33 (full form); a copy for ZUS, social assistance or maintenance matters is exempt from stamp duty. To start, 3-4 short-form copies are usually enough: bank, insurer, ZUS, notary. Missing copies can be ordered later at any civil registry office or via the mObywatel service.
